*FOR ALL PARTICIPANTS UNDER 18, THIS FORM MUST BE COMPLETED BY A LEGAL PARENT/GUARDIAN. THIS FORM MUST BE COMPLETED BY/FOR ALL PARTICIPANTS WHO WILL VOULENTEER AT THE 2023 ETHIOPIAN FALL FESTIVAL.*

Welcome to the 3rd Ethiopian Fall Festival which will be held on Sunday, October 8th, 2023 in Alexandria, VA. We are thrilled that you want to a be part of making our festival a special event for the community. In order to volunteer at the Fall Festival, you must submit a permission slip at your earliest convenience.

By clicking the submit button below, you (parent/ legal guardian/participant) agree and understand that it is the legal equivalent of your manual/handwritten signature and consent to be legally bound to this agreement. You acknowledge that ECSAC does not hold any liability for accidents or adverse reactions to food consumed during the time in which you or your child volunteers.
