*FOR ALL PARTICIPANTS UNDER 18, THIS FORM MUST BE COMPLETED BY A LEGAL PARENT/GUARDIAN. THIS FORM MUST BE COMPLETED BY/FOR ALL PARTICIPANTS WHO WILL JOIN THE CLASS.*
From 9/23/2023 - 12/17/2023, ECSAC will host an Art class from 2:00-3:15 PM followed by an Amharic classes from 3:30-4:45 PM every Saturday. In any case of emergency during either our class, the contact information below is what we will you to reach you.
Please be aware that there will be snacks involved in our class which could contain common allergens such as dairy, eggs, wheat, soybeans, tree nuts, peanuts, fish, shellfish or wheat. Additionally, students will have the opportunity to play with each other during breaks and at the end of classes.
By clicking the submit button below, you (parent/ legal guardian/participant) agree and understand that it is the legal equivalent of your manual/handwritten signature and consent to be legally bound to this agreement. You confirm that you have read and agree to the conditions of the Amharic/ Art class. You acknowledge that ECSAC does not hold any liability for accidents or adverse reactions to food consumed.
